How to Request an Additional Score Report

ETS can send scores on tests taken over the past ten years to additional school districts or agencies. The fee per score recipient is $40. Additional score reports will show only the highest score earned. Test takers receive a copy of the score report, confirming it was sent.

Request an additional score report by one of the following methods:

Mail

Download and complete the Additional Score Report Request form or submit a request in writing to ETS, including: name, address, telephone number, candidate ID number (if known), Social Security number, date of birth, designated score recipient(s), test date and the correct fee.
