As an authorized representative of my institution, I certify that the assessments will not be reviewed, used or disclosed for any commercial purpose, and that I will be responsible for the proper safeguarding of these confidential assessments, and I agree to:

  • • Keep access to assessments and information about assessments secure from unauthorized disclosure whenever assessments are not actually being reviewed.

  • • Permit review of assessments only by other staff members that have a legitimate need to review the assessments, and only in my continuous presence.

  • • Prohibit the making of notes, copies or transcripts of assessment content for any purpose.
